Insurance Plan

We accept all major insurances, with the exception of United Healthcare and UMR.

We are able to treat Medicaid patients who are under 21 years old at our clinic. However, for Medicaid patients over the age of 21, Arkansas Medicaid requires that therapy services be provided through hospital-based outpatient clinics. Unfortunately, this means we are not permitted to treat adult Medicaid patients at our facility due to state regulations. However, if Medicaid is your secondary insurance and you have a primary insurance that we accept, we are able to treat you at our facility.

You can receive outpatient therapy services directly without a referral. However, many insurance companies require a referral or script from a physician in order to pay for these services.

Prior to scheduling your first visit, we will ask to obtain your insurance plan(s) information. With this information, we will verify your insurance coverage and benefits. We will need to collect enough information to verify coverage. Once we verify coverage, we will provide you with an estimate of costs to you. We will bill your primary insurance company for you. Please be aware most insurances require a portion of each visit to be patient responsibility in the form of a co-pay. We require co-pays to be made at the time of service.

Elevation Therapy Services

Medicare

Medicare requires a referral from a physician for physical therapy, which is valid for the first 30 days of treatment. After that, a new referral is required every 30 days to continue therapy. If continued care is necessary, we will make every effort to obtain a new referral for you. Your physical therapist will re-evaluate your condition and send a report to your physician to authorize continued care. Your physician may ask you to schedule a visit with their office before authorizing further treatment.

Workman’s Comp

We do accept workman’s comp patients. Please be aware that your WKCP insurance/administrator must approve any therapy before we can begin and/or continue therapy, even with a physician referral.

Motor Vehicle Accidents

We do accept Motor Vehicle Accident (MVA) cases. Please note that your MVA insurance or claim administrator must authorize therapy before we can begin or continue treatment—even with a physician’s referral.

Self-Pay

We do offer therapy on a self-pay basis for those who do not have insurance, maxed out their insurance benefits, or do not want to use their insurance. Please do not hesitate to contact us at (501) 463-9057 for more information on our fees or with any questions.
